    Objetivo del estudio:

    • Evaluar la precisión y fiabilidad de las mediciones de la matriz de transimpedancia (TIM) para la evaluación intraoperatoria del posicionamiento de la matriz de electrodos durante la implantación coclear.
    • Comparar los datos del TIM con las imágenes radiográficas para detectar anomalías en la colocación de electrodos.
    • Investigar la utilidad de TIM en la identificación de configuraciones específicas como el plegado de la punta (TFO).

    Principales métodos:

    • Estudio observacional retrospectivo de 15 pacientes (24 orejas) sometidos a CI con conjuntos de electrodos CI532® y CI632®.
    • Se recopilaron y compararon datos de TIM intraoperatorios con imágenes radiográficas intraoperatorias e inmediatamente postoperatorias.
    • Se analizaron mapas de calor TIM para la distribución del gradiente de tensión y la detección de TFO.

    Principales resultados:

    • El 96% de las implantaciones cocleares mostraron una distribución regular de la tensión TIM, lo que indica una colocación modular adecuada.
    • Se identificó con precisión una sola instancia de doblez de punta (TFO) durante el procedimiento quirúrgico utilizando TIM y se confirmó por radiografía.
    • Los hallazgos de TIM fueron concordantes con las evaluaciones radiológicas en todos los demás casos, demostrando una alta fiabilidad.

    Conclusiones:

    • Las mediciones de la matriz de transimpedancia (TIM) son una herramienta intraoperatoria confiable para la implantación coclear con matrices de electrodos Slim Modiolar.
    • TIM detecta con precisión las desviaciones de colocación de electrodos, mejorando la precisión quirúrgica y reduciendo potencialmente la exposición a la radiación y las imágenes postoperatorias.
    • Se necesitan más estudios prospectivos para validar la implantación clínica del TIM y establecer protocolos estandarizados.
