Resumen

Las imágenes PET/CT del receptor de colecistoquinina-2 (CCK2R) con 68Ga-DOTA-MGS5 muestran altas tasas de detección de tumores en pacientes con cáncer medular de tiroides (MTC). Este nuevo enfoque PET/CT demostró un valor limitado en los tumores neuroendocrinos gastroenteropancreáticos (GEP-NET), pero un potencial en los NET broncopulmonares.

Sus antecedentes:

  • El cáncer medular de tiroides (MTC) y otros tumores neuroendocrinos (NET) a menudo se presentan con metástasis.
  • La imagen diagnóstica precisa es crucial para la estadificación y la planificación del tratamiento en pacientes con NET.
  • El receptor de colecistoquinina-2 (CCK2R) es un objetivo potencial para la obtención de imágenes de NET.

Objetivo del estudio:

  • Evaluar el rendimiento diagnóstico de CCK2R PET/CT utilizando 68Ga-DOTA-MGS5 en pacientes con TCM avanzada y otras TEN.
  • Comparar la tasa de detección de lesiones y la absorción de marcadores de 68Ga-DOTA-MGS5 PET/CT con las normas de PET/CT establecidas.

Principales métodos:

  • Un estudio piloto prospectivo de fase 1/2a incluyó a 6 pacientes con MTC y 6 pacientes con otras NET (4 GEP-NET, 2 BP-NET).
  • Todos los pacientes tenían metástasis conocidas confirmadas por 68Ga-DOTATOC y 18F-DOPA PET/CT.
  • El rendimiento diagnóstico se evaluó comparando la detección de lesiones y la absorción de marcadores (SUVmáx/SUVmedia) de 68Ga-DOTA-MGS5 PET/CT con el PET/CT estándar de referencia.

Principales resultados:

  • 68Ga-DOTA-MGS5 PET/CT mostró un aumento de la actividad de la piscina sanguínea pero un mejor contraste de la lesión 2 horas después de la inyección.
  • Cuatro de los 6 pacientes con MTC presentaron lesiones positivas; la detección de lesiones fue comparable a la de 18F-DOPA y 68Ga-DOTATOC.
  • En las NETs no MTC, 68Ga-DOTA-MGS5 PET/CT mostró potencial en 2 pacientes con BP-NET pero valor limitado en 4 pacientes con GEP-NET.

Conclusiones:

  • CCK2R PET/CT con 68Ga-DOTA-MGS5 demuestra una alta tasa de detección de lesiones tumorales en pacientes con MTC, con una absorción comparable a la de las imágenes estándar.
  • Esta modalidad de imagen puede ser aplicable a las NET broncopulmonares, lo que justifica una investigación adicional en cohortes más grandes.
  • El 68Ga-DOTA-MGS5 PET/CT parece tener una utilidad limitada en las NET gastroenteropancreáticas.
