The sense of smell is achieved through the activities of the olfactory system. It starts when an airborne odorant enters the nasal cavity and reaches olfactory epithelium (OE). The OE is protected by a thin layer of mucus, which also serves the purpose of dissolving more complex compounds into simpler chemical odorants. Humans detect odors with the help of specialized cells located in the upper part of the nasal cavity, called olfactory receptor neurons (ORNs). Resumen

La preservación del colgajo olfativo durante la cirugía transsphenoidal (TSA) ayuda a una recuperación más rápida del olfato. La función olfativa objetiva mejora más rápido que la percepción subjetiva del olfato después de la AST.

Sus antecedentes:

  • La cirugía transsfenoidal (TSA) es un procedimiento común para los tumores de la hipófisis y la base del cráneo.
  • La disfunción olfativa es una complicación conocida de la AST.
  • La optimización de las técnicas quirúrgicas para preservar la función olfativa es crucial.

Objetivo del estudio:

  • Evaluar el impacto de la preservación del colgajo olfativo durante la TSA en la recuperación olfativa.
  • Para comparar los plazos de recuperación de la función olfativa objetiva y subjetiva.
  • Evaluar la recuperación de la calidad de vida específica olfativa (QOL) después de la AST.

Principales métodos:

  • Análisis retrospectivo de pacientes sometidos a AST.
  • Comparación de los resultados de la función olfativa entre pacientes con y sin preservación del colgajo olfativo.
  • Se utilizaron pruebas olfativas objetivas (por ejemplo, UPSIT) y cuestionarios de olor subjetivos (por ejemplo, escalas QOL).

Principales resultados:

  • La preservación del colgajo olfativo se asoció con una recuperación más temprana de la función olfativa.
  • La función olfativa objetiva demostró una trayectoria de recuperación más rápida en comparación con la olfacción subjetiva.
  • Las puntuaciones QOL olfativas específicas y las olfativas sino nasales exhibieron patrones de recuperación distintos.

Conclusiones:

  • La preservación del colgajo olfativo es una consideración quirúrgica beneficiosa en TSA para promover la recuperación olfativa temprana.
  • La discrepancia en las tasas de recuperación entre la olfacción objetiva y subjetiva justifica una investigación adicional.
  • Las estrategias quirúrgicas deben tener como objetivo mejorar los resultados objetivos y subjetivos del olfato y la calidad de vida después de la AST.
