Sus antecedentes:

  • La alocnesis, picazón por estímulos que normalmente no picazón como el tacto ligero, carga significativamente a los pacientes con condiciones de picazón crónica.
  • Este síntoma, observado principalmente en la dermatitis atópica, es probablemente subdiagnosticado en varios trastornos crónicos de picazón.
  • El diagnóstico actual se basa en métodos como los filamentos de Von Frey, con una falta de cuestionarios validados.

Objetivo del estudio:

  • Para aclarar los mecanismos neurofisiológicos subyacentes aloknesis.
  • Para diferenciar el aloknesis del prurito mecánico fisiológico.
  • Resaltar la necesidad de estrategias terapéuticas y de investigación clínica de la aloknesis.

Principales métodos:

  • Revisión de la literatura existente sobre las vías de picazón, la sensibilización y los mecanorreceptores.
  • Análisis de las funciones de la vía mecánica marcada y la vía polimodal en la aloknesis.

Principales resultados:

  • La alocnesis es un fenómeno patológico relacionado con la sensibilización del prurito, distinto del prurito mecánico fisiológico.
  • La sensibilización periférica, que involucra a los mecanorreceptores de umbral bajo (LTMR) y las interneuronas espinales, juega un papel clave.
  • Tanto las vías mecánicas etiquetadas como las polimodales contribuyen, convergiendo en la vía GRP/GRPR.

Conclusiones:

  • La alocnesis implica interacciones complejas entre los mecanismos de sensibilización periféricos y potencialmente centrales.
  • La comprensión de la participación de la vía dual (mecánica etiquetada y polimodal) ofrece información sobre la transducción de picazón.
  • La reducción de la aloknesis debe ser un objetivo terapéutico, lo que requiere más investigación y investigación en ensayos clínicos.
