Sus antecedentes:

Los trastornos temporomandibulares (TMD) representan un desafío significativo en el diagnóstico y el tratamiento.
Los criterios de diagnóstico existentes para las EMT requieren una herramienta de evaluación de la gravedad más completa.
Los cinco síntomas principales (5Ts) de los criterios de diagnóstico de las EMT (DC/EMT) proporcionan una base para evaluar la gravedad de la EMT.

Objetivo del estudio:

Desarrollar y validar un modelo multidimensional para evaluar la gravedad de las DTM.
Ampliar el filtro 5Ts incorporando la duración, la frecuencia, la intensidad y la interferencia (5Ts-4D).
Evaluar la fiabilidad y validez de los 5Ts-4D en diversos grupos de pacientes con TMD.

Principales métodos:

El 5Ts-4D fue desarrollado añadiendo cuatro dimensiones al screener 5Ts existente.
Se reclutaron participantes sin TMD (NT), TMD intraarticular (IT), TMD relacionado con el dolor (PT) y TMD combinado (CT).
Se administraron los 5Ts-4D, GCPS, JFLS-8 y OHIP-TMD; se probó su fiabilidad y validez.

Principales resultados:

El 5Ts-4D demostró una fuerte consistencia interna (alfa de Cronbach 0.83-0.95) y fiabilidad (ICC 0.51-0.97).
La herramienta mostró diferencias significativas entre los grupos de DMT (PT, CT, IT frente a NT).
La validez convergente fue confirmada a través de fuertes correlaciones con JFLS-8 y OHIP-TMD.

Conclusiones:

El 5Ts-4D es un instrumento confiable y válido para evaluar los síntomas y la gravedad de la DMT.
Esta herramienta ofrece un enfoque sencillo para evaluar la DTM.
El 5Ts-4D puede contribuir a mejorar las estrategias de tratamiento para los pacientes con TMD.
