Sus antecedentes:

  • Febuxostat es un inhibidor de la xantina oxidasa para la gota y la hiperuricemia.
  • Aunque generalmente es seguro, se han relacionado con el febuxostat casos raros de daño hepático inducido por fármacos.

Objetivo del estudio:

  • Informar de un caso de lesión hepática después del inicio de febuxostat.
  • Para subrayar la importancia del control de la función hepática en pacientes tratados con febuxostat.

Principales métodos:

  • Informe de caso de un paciente de unos 50 años.
  • Se analizaron la presentación clínica, las pruebas de laboratorio y los hallazgos de la biopsia hepática.
  • Se ha documentado una respuesta a la interrupción del tratamiento con febuxostat y al tratamiento posterior con prednisona.

Principales resultados:

  • El paciente desarrolló fatiga, náuseas, ictericia y aminotransferasas elevadas después de cambiar a febuxostat.
  • La biopsia del hígado confirmó una lesión hepatocelular aguda moderada.
  • Las pruebas hepáticas mejoraron solo después de un mes de interrupción del tratamiento y posterior tratamiento con prednisona.

Conclusiones:

  • Este caso contribuye a la comprensión de la lesión hepática inducida por febuxostat.
  • Las presentaciones clínicas e histológicas de febuxostat DILI pueden ser variables.
  • La vigilancia de las pruebas hepáticas es esencial para los pacientes a los que se les receta febuxostat.
