Sus antecedentes:

  • La osificación heterotópica (HO) implica una formación ósea anormal en los tejidos blandos, que con frecuencia afecta a la cadera después de una lesión neurológica.
  • Las opciones de tratamiento para la HO de cadera van desde tratamientos conservadores hasta intervenciones quirúrgicas como el reemplazo total de cadera (THR).

Objetivo del estudio:

  • Para informar de un tratamiento quirúrgico exitoso de la osificación heterotópica grave de la cadera.
  • Evaluar la eficacia de la escisión quirúrgica y el THR con una prótesis de doble movilidad en la restauración de la función de la cadera.

Principales métodos:

  • Un informe de caso de un hombre adulto joven con Brooker grado 4 HO de la cadera izquierda.
  • Excisión quirúrgica del hueso heterotópico del acetábulo y el fémur proximal.
  • Sustitución total de cadera (THR) con una prótesis de doble movilidad.
  • Principales resultados:

    • El paciente presentaba una grave restricción del movimiento de la cadera debido a una extensa osificación heterotópica.
    • El seguimiento postoperatorio a los 6 meses y 1 año mostró mejoras significativas en el dolor.
    • Se observaron mejoras notables en el rango de movimiento y los resultados funcionales después de la cirugía.

    Conclusiones:

    • La extirpación quirúrgica de la osificación heterotópica combinada con el reemplazo total de cadera ofrece un tratamiento viable para la afectación severa de la cadera.
    • Las prótesis de movilidad dual pueden proporcionar resultados favorables en el manejo de reconstrucciones complejas de cadera después de HO.
