Sus antecedentes:

  • La quimioembolización transarterial convencional (TACE) para el carcinoma hepatocelular (HCC) tiene una tasa de respuesta objetiva de aproximadamente el 50% para el control local.
  • TILA-TACE, un nuevo TACE integrado en bicarbonato, demostró previamente una eficacia del 100% en el control local y regional de HCC no resecable.

Objetivo del estudio:

  • Validar la eficacia, la selectividad y la seguridad de TILA-TACE en la práctica clínica real para el tratamiento del CHC.
  • Evaluar los resultados de TILA-TACE combinado con radioterapia para el CCH avanzado, incluida la invasión macrovascular y las metástasis extrahepáticas.

Principales métodos:

  • Un total de 413 pacientes con CHC (estadios tempranos, intermedios y avanzados) fueron incluidos en este estudio de validación en el mundo real.
  • TILA-TACE se utilizó para el tratamiento primario del tumor, mientras que la radioterapia se empleó para la invasión macrovascular y las metástasis extrahepáticas.
  • Se registraron y analizaron sistemáticamente los efectos secundarios de TILA-TACE.

Principales resultados:

  • TILA-TACE logró una tasa de respuesta objetiva del 99, 01% para los tumores de CHC, con una tasa de respuesta completa del 72, 77%.
  • La radioterapia demostró una tasa de respuesta objetiva del 96,88% para el trombo tumoral.
  • La mediana de supervivencia de los pacientes con CHC en estadio avanzado fue de 27 meses, siendo las metástasis intrahepáticas la causa principal de muerte relacionada con el cáncer.

Conclusiones:

  • TILA-TACE es un nuevo tratamiento altamente eficaz y seguro para el control local del CHC, con un perfil de seguridad tolerable.
  • La combinación de TILA-TACE y radioterapia proporciona beneficios significativos en la supervivencia de los pacientes con CHC avanzado.
  • Se necesitan más investigaciones para abordar las metástasis intrahepáticas como causa de mortalidad después del tratamiento.
