Resumen

Los pacientes con lupus eritematoso sistémico (LES) hospitalizados con insuficiencia cardíaca derecha (IFR) tienen un riesgo significativamente mayor de mortalidad y resultados adversos. Este estudio pone de relieve la necesidad crítica de mejorar las estrategias de tratamiento de la IRH en pacientes con LES.

Sus antecedentes:

  • Los pacientes con lupus eritematoso sistémico (LES) presentan un mayor riesgo cardiovascular.
  • Los datos sobre los resultados hospitalarios para la insuficiencia cardíaca derecha (IFR) en pacientes con LES son limitados.

Objetivo del estudio:

  • Evaluar el impacto del LES en los resultados cardiovasculares en pacientes hospitalizados con IRH.
  • Comparar la mortalidad y la morbilidad en el hospital entre los pacientes con ERH con y sin LES.

Principales métodos:

  • Utilizó la base de datos nacional de muestras de pacientes hospitalizados 2016-2019.
  • Comparación de los resultados de 111 pacientes con ERR con pacientes sin ERL.
  • Ajustado para la demografía de los pacientes y las comorbilidades utilizando modelos de regresión multivariables.

Principales resultados:

  • Los pacientes con LES presentaron una mayor mortalidad hospitalaria (aOR 3,8) en comparación con los pacientes sin LES.
  • El LES se asoció con una mayor probabilidad de lesión renal aguda (aOR 2,61) y el uso de vasopresores (aOR 8,11).
  • No se encontraron diferencias significativas en el uso de ventilación mecánica, la duración de la estancia o los costos totales de hospitalización.

Conclusiones:

  • El lupus eritematoso sistémico está relacionado con peores resultados hospitalarios, incluida la mortalidad, en pacientes con insuficiencia cardíaca derecha.
  • Se necesita más investigación para comprender los mecanismos que impulsan estos resultados adversos y para desarrollar intervenciones específicas.
