Este estudio sobre las infecciones osteoarticulares por Candida en Turquía encontró que las especies de Candida no albicans son comunes, con variada resistencia al fluconazol. El desbridamiento quirúrgico mejora la recuperación, mientras que la diabetes mellitus la dificulta.

  • Los factores que influyen en la recuperación del paciente en estas infecciones complejas requieren una mayor aclaración.

Objetivo del estudio:

  • Investigar la epidemiología, las características clínicas y la resistencia al fluconazol de las especies de Candida en infecciones osteoarticulares en Turquía.
  • Identificar los factores asociados con la recuperación completa de estas infecciones.
  • Informar sobre la prevalencia de Candida auris en esta cohorte de pacientes.

Principales métodos:

  • Estudio retrospectivo multicéntrico de 73 pacientes adultos con infecciones osteoarticulares probadas o probables asociadas con Candida (2015-2025).
  • Datos recogidos sobre la presentación clínica, las especies de Candida causantes, la susceptibilidad antifúngica, las comorbilidades y los resultados del tratamiento.
  • Análisis multivariado utilizado para identificar los factores que influyen en la recuperación clínica.

Principales resultados:

  • La espondilodiscitis fue la presentación más común, seguida de infecciones falangiales de manos y pies.
  • Las especies de Candida no albicans (50,7%) fueron frecuentes, siendo la Candida parapsilosis la más común; se identificó Candida auris.
  • La resistencia al fluconazol fue mayor en especies no albicans (27%) que en Candida albicans (3%).
  • La diabetes mellitus (68,5%) fue una comorbilidad prevalente.
  • El desbridamiento quirúrgico aumentó significativamente las probabilidades de recuperación (aOR, 5.764), mientras que la diabetes mellitus disminuyó las probabilidades (aOR, 0.205).

Conclusiones:

  • Este estudio proporciona datos epidemiológicos clave y tasas de resistencia al fluconazol para las infecciones osteoarticulares por Candida en Turquía.
  • La intervención quirúrgica es un predictor positivo significativo de la recuperación.
  • La diabetes mellitus es un predictor negativo significativo de la recuperación completa en estos pacientes.
