Sus antecedentes:

  • Los pacientes con fibrilación auricular detectada por el dispositivo (FAD) presentan un menor riesgo de accidente cerebrovascular en comparación con aquellos con fibrilación auricular diagnosticada por ECG (FA).
  • La decisión de la anticoagulación oral en DDAF requiere un cuidadoso equilibrio entre los beneficios potenciales de la prevención del ictus y los riesgos inherentes de la hemorragia.

Objetivo del estudio:

  • Evaluar la eficacia y seguridad de edoxaban en comparación con la ausencia de anticoagulación en pacientes con fibrilación auricular detectada por el dispositivo.
  • Para evaluar los resultados compuestos de muerte, accidente cerebrovascular, embolia sistémica, infarto de miocardio y hemorragia mayor.

Principales métodos:

  • Se realizó un análisis de la proporción de ganancias en el conjunto de datos del ensayo NOAH-AFNET 6.
  • El análisis primario ordenó jerárquicamente los resultados: muerte por todas las causas, accidente cerebrovascular, embolia sistémica / pulmonar / infarto de miocardio y hemorragia mayor.
  • Los análisis adicionales incorporaron la muerte cardiovascular y los resultados informados por los pacientes; las probabilidades de victoria se calcularon para comparaciones indecisas.

Principales resultados:

  • La relación de ganancia comparando edoxaban con la ausencia de anticoagulación fue de 0,87 (IC del 95%: 0,68-1,10; P = 0,23), lo que indica que no hay diferencias significativas.
  • Una gran proporción de las comparaciones fueron indecisas (84,9%), con edoxaban ganando el 46% y el placebo el 54% de los casos restantes.
  • La muerte y la hemorragia mayor fueron los eventos más frecuentes observados en el ensayo.

Conclusiones:

  • Este análisis de la relación de ganancia no sugiere ninguna ventaja de edoxaban sobre la ausencia de anticoagulación en pacientes con DDAF.
  • El estudio integró eventos trombóticos, muerte y sangrado, con y sin consideraciones de calidad de vida.
  • La muerte y la hemorragia mayor se identificaron como los efectos adversos más comunes.
