Sus antecedentes:

  • La fiebre reumática aguda (FRA) precede a la enfermedad reumática del corazón (ERC) después de las infecciones estreptocócicas del grupo A.
  • Muchos diagnósticos de RHD carecen de un historial documentado de ARF, lo que requiere la comparación de la gravedad de RHD entre estos grupos.
  • Este estudio investiga la progresión y la regresión de la ERH en individuos con y sin un diagnóstico previo de FRA.

Objetivo del estudio:

  • Comparar la gravedad y la progresión de la enfermedad reumática del corazón (ERC) en individuos con y sin antecedentes documentados de fiebre reumática aguda (FRA).
  • Evaluar la etapa de RHD en el diagnóstico y el seguimiento utilizando las pautas de la Federación Mundial del Corazón de 2023.
  • Identificar las diferencias en las tasas de regresión y progresión de RHD entre las cohortes ARF y no ARF.

Principales métodos:

  • Auditoría retrospectiva de imágenes ecocardiográficas de 292 personas (≤20 años) diagnosticadas con ERH en el Territorio del Norte, Australia (2012-2021).
  • Estadificación de la RHD en el diagnóstico y el seguimiento (mediana de 46 meses) según las directrices del WHF de 2023.
  • Comparación de la etapa, progresión y regresión de la ERH entre individuos con antecedentes de FRA y aquellos sin FRA.

Principales resultados:

  • En el momento del diagnóstico, el grupo de ARF tenía más RHD de Estadio A (28,6% frente a 12,0%), mientras que el grupo sin ARF tenía más Estadio B (50,0% frente a 38,0%).
  • No se observaron diferencias significativas en la RHD avanzada (etapas C y D) entre los grupos (p=0,440).
  • La regresión de la RHD fue significativamente mayor en el grupo de ARF (46% frente a 28%, p=0,014), sin diferencias en la progresión del estadio (p=0,367).

Conclusiones:

  • La gravedad de la enfermedad cardíaca reumática al momento del diagnóstico varía, y las personas con antecedentes de ARF presentan una mayor probabilidad de regresión.
  • Los hallazgos apoyan el cribado ecocardiográfico en poblaciones de alto riesgo para la detección temprana de RHD y el tratamiento con profilaxis antibiótica.
  • Se necesita más investigación para dilucidar las razones detrás de las diferencias observadas en los resultados de RHD entre los grupos ARF y no ARF.
