Objetivo del estudio:
- Investigar la estructura de la comunidad microbiana intestinal y la respuesta inmune del huésped en pacientes con colonización o infección por MDRO.
- Identificar las firmas microbianas específicas asociadas con el estado de MDRO.
- Explorar la interacción entre la microbiota intestinal y la disfunción inmune en las enfermedades asociadas con MDRO.
Principales métodos:
- Un estudio de caso-control en el que participaron 88 pacientes infectados por MDRO, 100 pacientes colonizados por MDRO y 86 controles sanos.
- Se utilizó la secuenciación del gen 16S rRNA para analizar la composición microbiana intestinal.
- Se realizó un perfil de citoquinas séricas para evaluar el estado inmune del huésped.
Principales resultados:
- Las cohortes MDRO mostraron una diversidad microbiana reducida y estructuras comunitarias alteradas, con una disminución de bacterias beneficiosas y un aumento de patóbiontes como Enterococcus y Klebsiella.
- Se observó un cambio en los enterotipos intestinales de dominados por Bacteroides a dominados por Enterococcus en pacientes colonizados e infectados.
- La disfunción inmune fue evidente, con correlaciones que muestran bacterias beneficiosas asociadas con marcadores antiinflamatorios y patobiontes con citoquinas proinflamatorias.
Conclusiones:
- La colonización de MDRO y la infección inducen una disbiosis intestinal significativa y una disfunción inmune.
- Las interacciones entre la microbiota y el sistema inmune son críticas en la progresión de las afecciones asociadas con MDRO.
- La microbiota intestinal representa un objetivo prometedor para nuevos enfoques diagnósticos y terapéuticos para combatir la resistencia antimicrobiana.
