Resumen

Sus antecedentes:

  • La enfermedad arterial periférica (EAP) tiene un impacto significativo en la morbilidad y mortalidad de los pacientes.
  • La EAP de varios niveles a menudo presenta desafíos complejos para el tratamiento.
  • La revascularización híbrida (HYR) combina técnicas abiertas y endovasculares para el tratamiento de la EAP.

Objetivo del estudio:

  • Evaluar los resultados clínicos y técnicos a medio plazo de HYR en pacientes sintomáticos con EAP de varios niveles.
  • Evaluar el impacto de la diabetes y el tratamiento antiplaquetario dual en los resultados de HYR.

Principales métodos:

  • Análisis retrospectivo de 366 pacientes consecutivos sometidos a HYR (endarterectomía común de la arteria femoral con procedimientos endovasculares).
  • Los pacientes fueron tratados en dos centros vasculares.
  • El seguimiento se realizó durante una mediana de 55 meses.

Principales resultados:

  • En general, el 63% de los pacientes presentaron isquemia crónica que amenazaba las extremidades.
  • Los resultados a medio plazo incluyeron una tasa de revascularización de la lesión objetivo (TLR) del 29% y una tasa de amputación mayor del 9%.
  • La supervivencia sin amputación (SFA) fue del 58% durante el período de estudio.

Conclusiones:

  • HYR proporciona resultados a medio plazo aceptables para la EAP de varios niveles.
  • La diabetes tiene efectos adversos sobre el SFA, la permeabilidad primaria (PP) y la TLR.
  • El tratamiento antiplaquetario dual postoperatorio se asoció con PP y TLR favorables.
