Resumen
La resistencia del VIH a dolutegravir es más común cuando se usa con ciertos inhibidores de la transcriptasa inversa nucleosídica. Específicamente, las combinaciones que incluían lamivudina con zidovudina o abacavir mostraron tasas de resistencia más altas.

- En total, se observó resistencia al dolutegravir en 21 participantes (11,5%).
- Se asociaron tasas significativamente más altas de resistencia al dolutegravir con dolutegravir+lamivudina+zidovudina (OR=19.4, IC del 95% 5.1-74.3) y dolutegravir+lamivudina+abacavir (OR=5.4, IC del 95% 1.1-25.8).
- Estas tasas de resistencia se compararon con la combinación de dolutegravir+lamivudina+tenofovir.
Conclusiones:
- La resistencia al VIH-dolutegravir es más frecuente cuando se administra con NRTIs que poseen vidas medias intracelulares más cortas, como la zidovudina y el abacavir.
- La combinación de dolutegravir con lamivudina y tenofovir parece estar asociada con un menor riesgo de resistencia al dolutegravir.
- Estos hallazgos ponen de relieve la importancia de la selección de la columna vertebral NRTI para prevenir el surgimiento de la resistencia a los medicamentos del VIH.
