Sus antecedentes:

Objetivo del estudio:

  • Examinar los cambios objetivos y subjetivos en la documentación clínica tras la aplicación de las notas abiertas.
  • Evaluar el impacto de las notas abiertas en las prácticas de documentación en las clínicas psiquiátricas ambulatorias.

Principales métodos:

  • Análisis de 876 notas clínicas (antes y después de la aplicación) utilizando 16 características lingüísticas.
  • Entrevistas cualitativas con 10 profesionales de la salud psiquiátrica analizadas mediante análisis temático reflexivo.
  • Utilizado Wilcoxon firmó prueba de rango para el análisis lingüístico cuantitativo.

Principales resultados:

  • Después de la implementación, las notas mostraron una mayor comprensibilidad, orientación a los recursos y positividad emocional, con una disminución del lenguaje estigmatizante y degradante.
  • Los profesionales de la salud informaron de la adaptación de la documentación para centrarla más en el paciente, utilizando menos jerga y más explicaciones.
  • Los HCP experimentaron un aumento de la carga de trabajo y las demandas de tiempo debido a los ajustes del flujo de trabajo para las notas abiertas.

Conclusiones:

  • La aplicación de las notas abiertas conduce a cambios objetivos y subjetivos significativos en la documentación clínica de salud mental.
  • Los hallazgos sugieren que los profesionales de la salud buscan notas más amigables con el paciente, lo que podría beneficiar a los pacientes y a la alianza terapéutica.
  • Las notas abiertas sostenibles requieren una integración eficiente del flujo de trabajo y la educación de los profesionales de la salud sobre la documentación centrada en el paciente.
