Sus antecedentes:

  • La arritmia cardíaca a menudo ocurre junto con anomalías estructurales del corazón como la miocardiopatía y la fibrosis.
  • Existe una relación bidireccional donde las perturbaciones eléctricas y la remodelación estructural se exacerban mutuamente.
  • Los estudios genéticos existentes se centran principalmente en las variantes de los canales iónicos, dejando poco investigados los mecanismos moleculares de la patogénesis y la progresión de la arritmia.

Objetivo del estudio:

  • Investigar los mecanismos moleculares subyacentes a la patogénesis de la arritmia y la progresión a la enfermedad cardíaca estructural.
  • Identificar las variantes genéticas que predisponen a los pacientes a procesos inflamatorios y fibróticos asociados con arritmias.
  • Para explorar los cambios en la expresión génica específica del tipo de célula en los corazones con arritmias.

Principales métodos:

  • Se realizó la secuenciación de todo el exoma en 50 pacientes con arritmia, centrándose en mutaciones exónicas raras y dañinas.
  • Mutación recurrente (r.m.) Los genes se analizaron utilizando redes de interacción proteína-proteína y enriquecimiento de la ontología génica.
  • Los datos de secuenciación de ARN de una sola célula de corazones arritmicos y de control se integraron para examinar la expresión génica específica del tipo de célula, particularmente los estados de los macrófagos SPP1+.

Principales resultados:

  • 132 r.m. en el extranjero. Se identificaron genes en al menos el 30% de los pacientes, enriquecidos en la regulación inmune, la homeostasis tisular, la matriz extracelular y las vías de transporte vesicular.
  • El análisis de una sola célula reveló firmas transcripcionales conservadas de respuestas de citoquinas mejoradas y programas pro-fibrogénicos en todos los tipos de células.
  • Se encontraron determinantes genéticos para la activación de los macrófagos SPP1+, con mutaciones específicas como ADAMTS7 asociadas con la enfermedad de inicio temprano y SLC9B1/ OTOA con predominio femenino.

Conclusiones:

  • El análisis exploratorio destaca cómo las variantes genéticas pueden predisponer a los pacientes con arritmia a procesos inflamatorios y fibróticos.
  • Los hallazgos sugieren posibles mecanismos moleculares que vinculan la progresión de la arritmia con la enfermedad cardíaca estructural.
  • Las vías candidatas identificadas pueden orientar futuras investigaciones terapéuticas para afecciones cardíacas relacionadas con la arritmia.
