Objetivo del estudio:

  • Para comprobar si la depresión persistente está asociada con dificultades para revisar las expectativas basadas en información positiva de psicoterapia.
  • Investigar si la inmunización cognitiva interviene en la relación entre el subtipo de depresión y la revisión de expectativas.
  • Para comparar los cambios de expectativas en la depresión persistente frente a la episódica.

Principales métodos:

  • 54 pacientes ambulatorios con trastorno depresivo mayor persistente y 102 con trastorno depresivo mayor episódico participaron antes del tratamiento.
  • Los participantes vieron informes de resultados positivos de la psicoterapia.
  • El cambio en las expectativas de tratamiento de antes a después de la consulta fue la medida de resultado principal.

Principales resultados:

  • Los pacientes con depresión persistente informaron expectativas de tratamiento más bajas que los pacientes con depresión episódica.
  • Los pacientes con depresión persistente mostraron menos cambios en las expectativas después de ver los informes positivos.
  • Este efecto fue más pronunciado para las expectativas de resultado de la psicoterapia y en una condición que promueve la inmunización cognitiva.

Conclusiones:

  • Las personas con depresión persistente presentan un ajuste deficiente de las expectativas de tratamiento cuando se exponen a información psicoterapéutica positiva.
  • La dificultad para revisar las expectativas puede representar un factor de riesgo para los peores resultados del tratamiento en la depresión persistente.
  • La inmunización cognitiva juega un papel, lo que sugiere que incluso las dudas menores pueden impedir la integración de la información en la depresión persistente.
