Sus antecedentes:

  • La emulación del ensayo objetivo (ETC) proporciona un marco para la inferencia causal a partir de datos observacionales.
  • La replicación de ensayos clínicos aleatorizados (ECA) es crucial para validar el TTE en ámbitos de investigación específicos.
  • La evaluación de las terapias modificadoras de la enfermedad (TMA) para la esclerosis múltiple (EM) requiere metodologías sólidas.

Objetivo del estudio:

  • Replicar ocho ECA sobre la eficacia de la DMT en la EM utilizando datos del registro francés.
  • Evaluar la validez de la ETT en el contexto de la investigación sobre el tratamiento de la EM.
  • Comparar los efectos del tratamiento derivados de la TTE con los hallazgos de ECA establecidos.

Principales métodos:

  • Estudio observacional retrospectivo utilizando la base de datos francesa de la OFSEP (14.111 pacientes).
  • Emulación de ocho ECA específicos que evalúan varias DMT en los Estados Miembros.
  • Estimador de probabilidad máxima dirigido para la estimación del efecto del tratamiento, ajustado por factores de confusión, censura y datos faltantes.

Principales resultados:

  • Los resultados de TTE concordaron con los hallazgos de ECA en 7/8 ensayos para las tasas de recaída y 6/6 ensayos para la progresión de la discapacidad.
  • La replicación de los resultados radiológicos (lesiones T2, lesiones T1 mejoradas con gadolinio) mostró una menor concordancia (3/5 y 1/4 ensayos, respectivamente).

Conclusiones:

  • La metodología TTE, combinada con datos de registro de alta calidad, es un enfoque válido para evaluar la efectividad del tratamiento de la EM.
  • El TTE muestra un gran potencial para la generación de evidencia en el mundo real en la investigación de la esclerosis múltiple.
  • Puede ser necesario un mayor refinamiento para replicar resultados radiológicos complejos utilizando TTE.
