Sus antecedentes:

  • La regurgitación tricúspide funcional (fTR) implica una dinámica de flujo compleja.
  • La forma de la envoltura de onda continua Doppler (CWD) en fTR no se entiende completamente en todos los espectros de gravedad.
  • El corte de onda v es un marcador conocido para fTR grave, pero otras formas requieren exploración.

Objetivo del estudio:

  • Para explorar el espectro de formas de envolvente CWD en fTR.
  • Para investigar la asociación de las formas de CWD con la gravedad de fTR.
  • Evaluar el valor pronóstico de los parámetros de la forma de la CWD en pacientes con fTR.

Principales métodos:

  • 245 pacientes con fTR fueron sometidos a ecocardiografía.
  • La gravedad de TR se clasificó utilizando el área de superficie de isovelocidad proximal corregida.
  • Las formas de CWD se evaluaron cualitativa y cuantitativamente utilizando Vmax/Vmean y TTP/TRD.

Principales resultados:

  • La relación Vmax/Vmedia aumentó con la severidad de la fTR, de formas de corte parabólicas a triangulares a ondas v.
  • Vmax/Vmedio y TTP/TRD correlacionados con Vmax, EROA y función de RV.
  • Los tercios de Vmax/Vmedia más altos se asociaron de forma independiente con resultados adversos (muerte/hospitalización por HF).

Conclusiones:

  • La relación Vmax/Vmedia refleja efectivamente la severidad hidráulica de la fTR.
  • Este parámetro ofrece un predictor simple e independiente de los resultados clínicos adversos en fTR.
  • Vmax/Vmedia puede aumentar la estratificación del riesgo en pacientes con fTR.
