Resumen

La arteritis de Takayasu (TAK) afecta a las arterias grandes y presenta hallazgos ecográficos carotídeos distintivos como el signo del macarrones y el aumento del grosor íntima-media (GIM). Estos cambios se correlacionan con marcadores inflamatorios como la proteína C reactiva (PCR).

Sus antecedentes:

  • La arteritis de Takayasu (TAK) es una vasculitis rara de grandes vasos que afecta principalmente a la aorta y sus ramas.
  • La ecografía puede detectar un aumento del grosor íntima-media (GIM) y cambios ateroscleróticos en las arterias carótidas de pacientes con TAK.
  • Es crucial distinguir la TAK de otras afecciones con manifestaciones vasculares similares para un diagnóstico y manejo precisos.

Objetivo del estudio:

  • Evaluar las características ecográficas carotídeas detalladas en pacientes con arteritis de Takayasu (TAK).
  • Comparar estos hallazgos ecográficos con los de pacientes con diabetes mellitus (DM).
  • Evaluar la correlación entre los parámetros ecográficos carotídeos y los marcadores inflamatorios en la TAK.

Principales métodos:

  • Estudio de casos y controles que incluyó a 15 pacientes con TAK y 15 pacientes con DM.
  • La evaluación incluyó factores de riesgo cardiovascular, ecografía carotídea y marcadores inflamatorios séricos (VSG, PCR).
  • Los datos se analizaron mediante pruebas de regresión univariante y múltiple.

Principales resultados:

  • Se observaron placas ateroscleróticas en el 46,7% de los pacientes con TAK y en el 53,3% de los pacientes con DM.
  • Los pacientes con TAK presentaron un GIM significativamente mayor en las arterias carótidas comunes (ACC) y internas (ACI).
  • Los hallazgos característicos en la TAK incluyeron el signo del macarrones, flujo turbulento y un mayor índice de resistencia (IR) de la ACC.
  • El GIM se correlacionó con los niveles de PCR.

Conclusiones:

  • La ecografía carotídea es una herramienta diagnóstica valiosa para la arteritis de Takayasu (TAK).
  • Las características ecográficas distintivas clave de la TAK incluyen el signo del macarrones, el flujo turbulento y el aumento del GIM y el IR.
  • El aumento del GIM carotídeo en la TAK se asocia con niveles elevados de proteína C reactiva (PCR).
