  • Se exploran los niveles urinarios de 25-hidroxivitamina D (25(OH)D) como un biomarcador potencial para el riesgo de ITU.

Objetivo del estudio:

  • Investigar la asociación entre los niveles urinarios de 25(OH)D y la ocurrencia de ITU.
  • Desarrollar y evaluar modelos de aprendizaje automático para la predicción de ITU utilizando datos demográficos y bioquímicos.

Principales métodos:

  • Análisis de una cohorte de 358 sujetos con datos demográficos, bioquímicos y microbiológicos recopilados.
  • Desarrollo de 12 modelos de aprendizaje automático que utilizan 25(OH)D urinaria, pH de orina, edad y género.
  • Los modelos se evaluaron utilizando métricas que incluyen precisión, sensibilidad, especificidad, VPP, VPN, AUC-ROC y puntuación F1.

Principales resultados:

  • Se observaron diferencias significativas en los niveles urinarios de 25(OH)D entre individuos con urocultivos positivos y negativos (p < 0,001).
  • Los modelos de aprendizaje automático alcanzaron precisiones que oscilaron entre el 64 % y el 87 %, y un modelo de apilamiento alcanzó el 88 % de precisión.
  • El modelo de apilamiento de mejor rendimiento demostró una sensibilidad del 83 %, una especificidad del 94 % y un AUC-ROC de 0,93.

Conclusiones:

  • Los niveles más bajos de 25(OH)D urinaria se asocian significativamente con una mayor ocurrencia de ITU.
  • Los modelos de aprendizaje automático muestran ser prometedores como herramientas complementarias para el diagnóstico de ITU, complementando los métodos tradicionales.
  • Se necesitan más estudios de validación y prospectivos para evaluar el impacto clínico de estos modelos predictivos.
