Sus antecedentes:

  • La enfermedad cardiovascular (ECV) es una de las principales causas de muerte a nivel mundial.; Las herramientas existentes de riesgo cardiovascular (CVR) son limitadas para poblaciones únicas como los mineros.; Este estudio utiliza el aprendizaje automático (ML) y datos longitudinales para predecir la progresión del CVR en entornos ocupacionales.

Objetivo del estudio:

  • Desarrollar modelos de ML para predecir la progresión del riesgo cardiovascular utilizando marcadores clínicos accesibles.; Evaluar la utilidad del Índice de Masa Corporal (IMC) y la Glucosa en Sangre (BG) como proxies de CVR en trabajadores de la minería.; Abordar las limitaciones de las herramientas actuales de evaluación de CVR en condiciones de trabajo extremas.

Principales métodos:

  • Análisis longitudinal retrospectivo de datos de salud de 89.045 trabajadores mineros chilenos.; Modelado de transiciones entre categorías definidas de IMC y BG utilizando pares de visitas sucesivas.; Aplicación de técnicas de ML (XGB, RF) con validación cruzada estratificada y ajuste de hiperparámetros.; Evaluación del rendimiento del modelo utilizando AUC, precisión, sensibilidad y especificidad.

Principales resultados:

  • Los modelos de ML lograron una alta precisión en la predicción de transiciones de IMC, con AUC de hasta 0.95 para progresión severa (obesidad mórbida).; La predicción de transición de glucosa en sangre (BG) mostró resultados accionables, con AUC de hasta 0.83 para progresión a diabetes.; Los modelos demostraron buena generalización y consistencia, con mínima evidencia de sobreajuste.

Conclusiones:

  • Los modelos de ML predicen eficazmente transiciones de riesgo de IMC y BG clínicamente relevantes a partir de datos de salud ocupacional.; Los datos longitudinales y la evaluación basada en escenarios mejoran el rendimiento de los modelos de ML para la evaluación de CVR.; Este enfoque ofrece potencial para mejorar la evaluación de CVR y la toma de decisiones preventivas en poblaciones de trabajo de alto riesgo.
