Sus antecedentes:

  • El cáncer de tiroides (TC) es una neoplasia endocrina común.
  • La recurrencia del TC representa un desafío clínico significativo, que afecta el pronóstico del paciente.
  • Los modelos estadísticos tradicionales luchan por predecir con precisión la recurrencia del TC.

Objetivo del estudio:

  • Desarrollar un marco novedoso de aprendizaje de conjunto de apilamiento para predecir la recurrencia del TC.
  • Mejorar la precisión y la interpretabilidad de la predicción de la recurrencia del TC.
  • Crear una herramienta fácil de usar para el apoyo a la toma de decisiones clínicas.

Principales métodos:

  • Se utilizó un conjunto de datos de 383 pacientes (108 de recurrencia, 275 de no recurrencia).
  • Un marco de conjunto de apilamiento integró SGD, ET y DT con XGBoost como meta-aprendiz.
  • Se empleó el método SHAP para la interpretabilidad del modelo y la identificación de factores.

Principales resultados:

  • El modelo de apilamiento logró una precisión del 96,52%, una puntuación F1 del 93,55% y un AUC de 0,9921.
  • Principales predictores identificados: respuesta al tratamiento, edad, estadio N, estratificación del riesgo y adenopatía.
  • Se desarrolló una herramienta web interactiva, TCCheck, para la predicción en línea.

Conclusiones:

  • El marco desarrollado es eficaz e interpretable para la predicción de la recurrencia del TC.
  • La herramienta TCCheck proporciona soporte para la toma de decisiones clínicas explicable e individualizada.
  • El marco sirve como referencia para la predicción de recurrencia en otros cánceres.
