Sus antecedentes:

  • Los tumores intratorácicos gigantes representan un desafío clínico significativo debido a su tamaño y ubicación.; Estos tumores a menudo involucran múltiples compartimentos mediastínicos y cavidades pleurales, lo que complica el manejo.

Objetivo del estudio:

  • Analizar los abordajes quirúrgicos y los resultados en pacientes con tumores intratorácicos gigantes.; Evaluar la efectividad de diferentes técnicas de acceso quirúrgico en la resección de estas masas complejas.

Principales métodos:

  • Revisión retrospectiva de 40 pacientes con tumores intratorácicos gigantes tratados entre 1982 y 2024.; Análisis de tipos de tumores, métodos de acceso quirúrgico (esternotomía, toracotomía, hemiclamshell, combinada) y resultados postoperatorios.; Inclusión de datos sobre biopsias preoperatorias e intentos de resección previos.

Principales resultados:

  • El estudio incluyó 40 pacientes con diversos tipos de tumores, incluyendo teratomas y tumores fibrosos solitarios.; La esternotomía fue el abordaje quirúrgico más común (25 casos), seguida de la toracotomía lateral (7 casos).; La morbilidad postoperatoria fue del 17,5% con una tasa de mortalidad del 7,5%, mientras que el 82,5% de los pacientes se recuperaron bien.

Conclusiones:

  • La resección quirúrgica es factible para tumores intratorácicos gigantes, con diversos abordajes que arrojan resultados aceptables.; La cuidadosa selección del acceso quirúrgico es crucial para el manejo de estas neoplasias torácicas complejas.; A pesar de los desafíos, la gran mayoría de los pacientes experimentan una recuperación exitosa después de la intervención quirúrgica.
