Resumen

Este estudio desarrolló un modelo transparente de Máquina de Impulso Explicable (EBM) para predecir la presión arterial sistólica (PAS) en pacientes en hemodiálisis, logrando una precisión y generalización competitivas. El modelo ofrece transparencia a nivel de características, lo que ayuda a comprender la variabilidad de la presión arterial para una atención personalizada.

Sus antecedentes:

  • La hipotensión y la hipertensión intradiálisis aumentan el riesgo cardiovascular en pacientes en hemodiálisis.
  • Los modelos actuales de predicción de la presión arterial sistólica (PAS) tienen limitaciones, incluidos rangos de error consistentes y falta de transparencia.
  • Una mejor monitorización de la PAS puede mejorar la atención al paciente más allá de la clasificación tradicional de eventos.

Objetivo del estudio:

  • Desarrollar un modelo de predicción de PAS utilizando Explainable Boosting Machine (EBM).
  • Evaluar la generalización del modelo EBM desarrollado en diferentes sucursales hospitalarias.
  • Comparar el rendimiento del modelo EBM con otros métodos comunes de aprendizaje automático.

Principales métodos:

  • Análisis retrospectivo de datos de 524 pacientes en hemodiálisis (2016-19).
  • Se utilizó Explainable Boosting Machine (EBM) para la predicción de PAS, incorporando parámetros de hemodiálisis, signos vitales y mediciones predialíticas.
  • Se empleó validación cruzada entre sucursales para la evaluación de la generalización y la selección de características, comparando EBM con otros cinco modelos de aprendizaje automático.

Principales resultados:

  • El modelo EBM logró un rendimiento competitivo (MAE: 10,57-11,33 mmHg, r: 0,80-0,83) en la validación cruzada.
  • Un gráfico de cascada visualizó las contribuciones de las características a la predicción de la PAS, ofreciendo transparencia.
  • Se encontraron altas correlaciones (r: 0,81-0,87) entre la variabilidad de la presión arterial y los errores de predicción.

Conclusiones:

  • Los modelos EBM demostraron una excelente generalización entre sucursales y transparencia a nivel de características, alineándose con la comprensión clínica.
  • La naturaleza transparente de EBM contrasta con los modelos de caja negra, lo que reduce la necesidad de métodos de explicación post-hoc.
  • Los hallazgos respaldan el desarrollo de soluciones específicas para el paciente para el manejo de patrones de presión arterial altamente variables en hemodiálisis.
