Sus antecedentes:

  • El síndrome post-neumonectomía (SPN) es una complicación rara después de la cirugía de extirpación de un pulmón.
  • Específicamente, el SCN puede surgir de cambios anatómicos únicos que afectan el corazón.
  • Este informe se centra en un paciente que presenta SCN debido a anomalías diafragmáticas.

Objetivo del estudio:

  • Describir un caso único de síndrome cardíaco post-neumonectomía.
  • Ilustrar los hallazgos diagnósticos asociados con esta rara condición.
  • Presentar una estrategia exitosa de manejo quirúrgico.

Principales métodos:

  • Se realizó una neumonectomía intrapericárdica derecha a un hombre de 56 años para un carcinóide atípico.
  • Postoperatoriamente, el paciente desarrolló disnea progresiva atribuida a la elevación del hemidiafragma derecho y al efecto de masa cardíaco.
  • La intervención quirúrgica implicó re-toracotomía, plicatura diafragmática y colocación de expansor de tejido.

Principales resultados:

  • La imagenología reveló una elevación significativa del hemidiafragma derecho que causaba un efecto de masa sobre el corazón.
  • No se identificaron anomalías traqueobronquiales como causa de los síntomas.
  • La intervención quirúrgica corrigió con éxito el defecto anatómico y resolvió la disnea del paciente.

Conclusiones:

  • El SCN puede ser el resultado de anomalías diafragmáticas después de una neumonectomía.
  • La re-toracotomía con plicatura diafragmática y reemplazo de volumen es un tratamiento eficaz.
  • El diagnóstico y la corrección quirúrgica tempranos son cruciales para el manejo del SCN.
