Resumen

La neumonía lipoidea, una afección pulmonar que imita a los tumores, requiere un diagnóstico cuidadoso. La identificación de lesiones que contienen grasa y el historial de exposición a lípidos ayudan a diferenciarla de las neoplasias pulmonares, evitando procedimientos innecesarios.

Sus antecedentes:

  • La neumonía lipoidea es una enfermedad pulmonar rara caracterizada por la acumulación de lípidos en los alvéolos.
  • El diagnóstico puede ser un desafío debido a su similitud radiográfica y clínica con los tumores pulmonares.

Objetivo del estudio:

  • Mejorar la precisión diagnóstica de la neumonía lipoidea.
  • Diferenciar la neumonía lipoidea de las neoplasias pulmonares malignas.

Principales métodos:

  • Análisis retrospectivo de cinco casos de sospecha de neoplasias evaluados mediante TC (2022-2024).
  • Criterios de inclusión: factores de riesgo oncológico e historial de exposición prolongada a aceite.
  • Revisión de pruebas tomográficas, hallazgos histológicos y factores de riesgo del paciente.

Principales resultados:

  • Los casos involucraron a adultos mayores (edad promedio 78 años) con factores de riesgo que incluyen tabaquismo y exposición crónica al aceite.
  • Los hallazgos de la TC variaron: consolidaciones, nódulos, masas y cavitaciones con calcificaciones.
  • La histopatología confirmó vacuolas intracitoplásmicas, características de macrófagos cargados de lípidos.

Conclusiones:

  • La neumonía lipoidea debe considerarse en adultos mayores con opacidades pulmonares persistentes, especialmente aquellos con antecedentes de exposición a lípidos.
  • La atenuación de grasa en la TC y una historia clínica completa del paciente son cruciales para un diagnóstico preciso.
  • Distinguir la neumonía lipoidea de los tumores puede prevenir procedimientos invasivos innecesarios.
