Resumen

Un modelo de aprendizaje profundo que utiliza secuencias de RM predice eficazmente el carcinoma hepatocelular (CHC) con mutación P53. El modelo combinado, que integra la fase arterial (FA), la fase venosa portal (FVP) y la imagen ponderada en T2 (T2WI), mostró una alta precisión en la identificación de esta variante de cáncer agresivo.

Sus antecedentes:

  • El carcinoma hepatocelular (CHC) con mutación P53 es agresivo, está relacionado con el factor de crecimiento endotelial vascular (VEGF) y una mayor densidad microvascular.
  • La predicción del estado de mutación P53 en el CHC es crucial para las terapias dirigidas y la mejora de los resultados del paciente.

Objetivo del estudio:

  • Desarrollar y evaluar un modelo de aprendizaje profundo que utilice datos de RM para la predicción no invasiva de CHC con mutación P53.
  • Comparar el rendimiento de secuencias de RM únicas frente a secuencias combinadas en la predicción del estado de mutación P53.

Principales métodos:

  • Análisis retrospectivo de 312 pacientes con CHC confirmado patológicamente que se sometieron a RM con contraste de gadolinio.
  • Desarrollo de un modelo de aprendizaje profundo basado en EfficientNetV2 utilizando secuencias de fase arterial (AP), fase venosa portal (VP), imagen ponderada en T2 (T2WI) y fase hepatobiliar (HBP).
  • Evaluación del rendimiento del modelo utilizando AUC, precisión, sensibilidad, especificidad, precisión y puntuación F1, con la prueba de Delong para comparaciones de AUC.

Principales resultados:

  • El modelo combinado multifásico (T2WI+AP+VP) superó significativamente a los modelos de secuencia única, logrando un AUC de 0.914 en el conjunto de datos de prueba.
  • El modelo de fase hepatobiliar (HBP) mostró el mejor rendimiento entre los modelos de secuencia única (AUC=0.715).
  • La incorporación de HBP en el modelo combinado no mejoró significativamente el rendimiento predictivo (P>0.05).

Conclusiones:

  • Un modelo de aprendizaje profundo que integra secuencias de RM T2WI, AP y VP es altamente efectivo para predecir el CHC con mutación P53.
  • Este enfoque impulsado por IA ofrece una herramienta no invasiva prometedora para identificar variantes agresivas de CHC.
  • Investigaciones futuras pueden explorar secuencias o características adicionales para mejorar potencialmente la precisión predictiva.
