Resumen

El novedoso método RAFF (Referring to the Anterior, Fixing on the Frontal) para la reparación de la válvula mitral utilizando cuerdas artificiales reduce significativamente la regurgitación residual y el abombamiento de los velos. Esta técnica mejora la precisión anatómica y la reproducibilidad para una competencia mitral duradera.

Objetivo del estudio:

  • Evaluar la utilidad clínica del novedoso método RAFF (Referring to the Anterior, Fixing on the Frontal) para la reparación de la válvula mitral.
  • Evaluar el impacto de la técnica RAFF en la precisión anatómica, la reproducibilidad y los resultados clínicos.
  • Comparar el método RAFF con las técnicas convencionales en términos de insuficiencia mitral y función de los velos.

Principales métodos:

  • Estudio retrospectivo de 67 pacientes sometidos a reparación electiva de la válvula mitral con cuerdas artificiales.
  • Comparación entre el grupo de técnica RAFF (n=36) y un grupo no RAFF (n=31).
  • La técnica RAFF implica la referencia de las cuerdas del velo anterior y la fijación del músculo papilar frontal para neochordas.

Principales resultados:

  • El grupo RAFF mostró significativamente menos regurgitación mitral residual postoperatoria (p=0,014).
  • El uso del método RAFF se asoció con una reducción del abombamiento de los velos postoperatorios y una menor recurrencia de insuficiencia mitral moderada/grave.
  • Se observaron una mayor longitud de coaptación, un menor tiempo de pinzamiento aórtico y ningún efecto hemodinámico adverso con RAFF.

Conclusiones:

  • La técnica RAFF proporciona un enfoque estandarizado y anatómicamente guiado para la reconstrucción de cuerdas artificiales.
  • Minimiza la variabilidad entre cirujanos y mejora la coaptación de los velos para una reparación mitral duradera.
  • El método RAFF ofrece una alternativa segura y eficaz para la reparación de la válvula mitral, mejorando los resultados clínicos.
