  • La heterogeneidad de la enfermedad de Alzheimer (EA) y los factores no relacionados con la EA complican el desarrollo terapéutico.; Trabajos previos identificaron discordancia entre tau (T) y neurodegeneración (N) utilizando imágenes multimodales.; Este estudio simplificó el enfoque utilizando p-tau217 en plasma y morfometría del lóbulo temporal medial (CMR).

Objetivo del estudio:

  • Investigar los patrones de desajuste tau-neurodegeneración (T-N) en el CMR utilizando p-tau217 en plasma y RM.; Identificar grupos de pacientes distintos basados en la discordancia T-N.; Explorar las implicaciones clínicas y cognitivas de estos grupos de desajuste T-N.

Principales métodos:

  • Se incluyeron 349 participantes de ADNI con T1-RM y p-tau217 en plasma.; Se segmentaron las regiones del CMR en subregiones y se parcelaron en superpuntos.; Se calcularon los residuos T-N y se utilizó agrupamiento ponderado para identificar grupos.

Principales resultados:

  • Fuerte asociación encontrada entre p-tau217 y atrofia del CMR.; Se identificaron tres grupos de desajuste T-N: canónico (N∼T), vulnerable (N>T) y resiliente (N

Conclusiones:

  • El desajuste T-N en el CMR utilizando RM y biomarcadores en plasma identificó grupos de pacientes con variabilidad en vulnerabilidad/resiliencia.; Los patrones del grupo vulnerable sugieren cambios neuropatológicos asociados a la TDP-43 predominantes en el sistema límbico relacionados con la edad (LATE-NC).; Este método ofrece un enfoque menos invasivo y rentable para estratificar individuos para intervenciones terapéuticas.
