  • La conexión entre el dolor crónico y la patogénesis de la enfermedad de Alzheimer (AD) no se comprende bien.; La proteómica basada en espectrometría de masas ofrece un método de alto rendimiento para analizar proteínas plasmáticas.; La identificación de perfiles de proteínas puede revelar vínculos entre el dolor crónico y la AD, sirviendo como marcadores de diagnóstico temprano.

Objetivo del estudio:

  • Identificar perfiles de proteínas plasmáticas asociadas con el riesgo de demencia en personas con dolor crónico.; Explorar posibles vías moleculares compartidas entre el dolor crónico y diversos tipos de demencia.; Descubrir nuevos biomarcadores plasmáticos para la detección temprana de demencia en pacientes con dolor crónico.

Principales métodos:

  • Análisis de 2.920 proteínas plasmáticas en 20.932 individuos con dolor crónico del UK Biobank.; Evaluación longitudinal de las asociaciones entre proteínas y el riesgo de demencia por todas las causas (ACD), AD y demencia vascular (VaD) utilizando modelos de riesgos proporcionales de Cox.; Análisis de enriquecimiento de la Kyoto Encyclopedia of Genes and Genomes (KEGG) y Gene Ontology (GO) para identificar vías biológicas.

Principales resultados:

  • GFAP mostró la asociación más fuerte con todos los tipos de demencia (ACD, AD, VaD).; Proteínas específicas como NEFL y GDF15 se identificaron como factores de riesgo para diferentes tipos de demencia, mientras que otras mostraron efectos protectores.; Los análisis de vías revelaron un enriquecimiento compartido en la interacción citocina-receptor de citocinas y la señalización Pi3k-akt, lo que sugiere un papel de la neuroinflamación.

Conclusiones:

  • Firmas distintas de proteínas plasmáticas vinculan el dolor crónico con un mayor riesgo de demencia.; GFAP es un marcador significativo en todos los tipos de demencia estudiados.; Los hallazgos sugieren que el dolor crónico puede acelerar la neurodegeneración a través de la neuroinflamación, ofreciendo objetivos para la intervención temprana y el desarrollo de biomarcadores.
