  • La disminución de las hormonas sexuales con el envejecimiento afecta la salud cerebrovascular y el riesgo de demencia. La menopausia temprana es un factor de riesgo significativo de demencia. El estradiol beneficia la vasculatura y la salud cerebral de las mujeres, pero sus efectos cerebrovasculares están poco estudiados; el papel de la testosterona en los hombres no está claro.

Objetivo del estudio:

  • Investigar las asociaciones específicas del sexo y la etapa de la vida entre las hormonas sexuales circulantes y los indicadores de salud cerebrovascular. Comprender cómo el estradiol y la testosterona influyen en el flujo sanguíneo cerebral (CBF), el tiempo de tránsito arterial (ATT) y la reactividad cerebrovascular (CVR) en relación con el envejecimiento y el estado menopáusico.

Principales métodos:

  • Se reclutaron 187 mujeres (reproductivas, perimenopáusicas, posmenopáusicas) y 157 hombres (jóvenes, mayores). La RM cuantificó el CBF, la ATT y la CVR. Las muestras de sangre midieron los niveles de hormonas sexuales circulantes; los análisis estadísticos (ANCOVA, regresiones polinomiales) examinaron las relaciones hormona-cerebrovascular.

Principales resultados:

  • En mujeres, el estradiol mostró relaciones inversas con la ATT (perimenopáusica), mientras que la testosterona tuvo asociaciones variadas con la CVR, ATT, CBF y CVR en las etapas reproductiva, perimenopáusica y posmenopáusica. En hombres, la testosterona se correlacionó positivamente con el CBF en individuos más jóvenes, mientras que el estradiol mostró una correlación inversa con el CBF en hombres mayores.

Conclusiones:

  • Las hormonas sexuales exhiben asociaciones distintas con los marcadores de salud cerebrovascular (CBF, ATT, CVR) que dependen del sexo y la etapa de la vida. El estradiol generalmente se correlacionó con una mejor salud cerebrovascular en las mujeres, mientras que la testosterona mostró efectos mixtos. La testosterona benefició el CBF de los hombres jóvenes, pero el estradiol afectó negativamente el CBF de los hombres mayores, lo que resalta el potencial de intervenciones hormonales dirigidas en la reducción del riesgo de demencia.
