Objetivo del estudio:

  • Investigar la microestructura del hipocampo como biomarcador de resiliencia cognitiva en la EA.
  • Examinar si la difusividad media del hipocampo (MD) modera la relación entre la carga de tau y el rendimiento cognitivo.
  • Comparar la eficacia de las métricas de DWI frente al volumen del hipocampo en la evaluación de la resiliencia cognitiva.

Principales métodos:

  • Se analizaron 192 participantes (14 con deterioro cognitivo leve) del Estudio BIOCARD.
  • La tomografía por emisión de positrones (PET) midió la carga in vivo de tau y beta-amiloide (Aβ).
  • La regresión lineal múltiple analizó el efecto moderador de la MD del hipocampo sobre la carga de tau y las puntuaciones cognitivas, controlando las covariables.

Principales resultados:

  • Una menor MD del hipocampo atenuó significativamente la asociación negativa entre la carga de tau y la cognición global (p < 0,006).
  • Se encontró una moderación similar para la memoria episódica y el lenguaje, pero no para la función ejecutiva o las habilidades visoespaciales.
  • El volumen del hipocampo no mostró efectos moderadores significativos (p > 0,150).

Conclusiones:

  • La microestructura mejorada del hipocampo es un biomarcador potencial de resiliencia cognitiva en la EA.
  • Las métricas de DWI, a diferencia de las medidas volumétricas, ofrecen información valiosa sobre los mecanismos cerebrales.
  • Estos hallazgos respaldan la promesa de DWI como medida de resultado para intervenciones tempranas en la EA.
