  • El dolor crónico afecta al 20% de los adultos mayores, lo que aumenta el riesgo de demencia por enfermedad de Alzheimer (AD).; Existe una investigación limitada sobre la asociación del dolor crónico con los biomarcadores de AD, particularmente para el dolor de moderado a severo clínicamente significativo.

Objetivo del estudio:

  • Investigar la asociación entre el dolor crónico de moderado a severo y el deterioro cognitivo y los biomarcadores de AD.; Explorar los roles mediadores del amiloide beta (Aβ) y la neurodegeneración en esta relación.

Principales métodos:

  • Se analizaron 2487 participantes de la Iniciativa de Neuroimagen de la Enfermedad de Alzheimer (ADNI) sin demencia basal.; Los participantes se categorizaron según el estado de dolor crónico de moderado a severo.; Se evaluó la función cognitiva, los biomarcadores de AD (Aβ42 de LCR, tau) y la RM estructural utilizando modelos mixtos y de mediación.

Principales resultados:

  • El dolor crónico de moderado a severo se correlacionó con un declive acelerado en la cognición global, la memoria, la función ejecutiva y el volumen cerebral total.; Se observó una acumulación más rápida de patología Aβ en individuos con dolor crónico de moderado a severo.; La patología Aβ y la pérdida de volumen cerebral total mediaron el vínculo entre el dolor y el declive cognitivo.

Conclusiones:

  • El dolor crónico de moderado a severo contribuye significativamente al deterioro cognitivo y la neurodegeneración general, mediado principalmente por Aβ.; Los hallazgos enfatizan la necesidad de considerar la gravedad del dolor crónico en la evaluación del riesgo de AD y las estrategias terapéuticas.
