Sus antecedentes:

  • El Deterioro Cognitivo Subjetivo (SCD) en individuos cognitivamente no afectados (CU) es un factor de riesgo para la progresión de la enfermedad de Alzheimer (EA).; La investigación actual sobre SCD se centra principalmente en las preocupaciones de memoria, descuidando otros fenotipos de EA como la Atrofia Cortical Posterior (PCA).; Existe la necesidad de métodos para identificar individuos con EA preclínica que puedan desarrollar variantes atípicas de EA como PCA.

Objetivo del estudio:

  • Investigar la asociación entre el deterioro visoespacial subjetivo y la función cognitiva y los biomarcadores en individuos CU.; Explorar la utilidad de la subescala visoespacial de la Everyday Cognition Scale (ECOG) para identificar la patología temprana de la EA.; Determinar si las preocupaciones visoespaciales subjetivas pueden predecir el desarrollo de variantes atípicas de la EA.

Principales métodos:

  • Análisis de las respuestas de autoinforme en la subescala visoespacial del ECOG de 253 participantes CU.; Exploración de las asociaciones entre las puntuaciones visoespaciales del ECOG, las pruebas cognitivas objetivas y la positividad de PET de amiloide (PiB+).; Realización de un modelo lineal general de PET de tau en todo el córtex para examinar el vínculo entre el deterioro visoespacial subjetivo y la carga de tau.

Principales resultados:

  • Cuatro por ciento de los participantes CU informaron dificultades visoespaciales, que no se relacionaron con la edad, el sexo, la educación o las puntuaciones del MMSE.; Las puntuaciones visoespaciales del ECOG no se correlacionaron con la cognición visoespacial objetiva ni con el MMSE.; Las preocupaciones visoespaciales se asociaron con una mayor carga de amiloide (PiB+) y deposición de tau en el hemisferio derecho en regiones implicadas en la EA preclínica.

Conclusiones:

  • El deterioro visoespacial subjetivo, poco común en el envejecimiento saludable, está presente en un subconjunto de individuos CU.; La subescala visoespacial del ECOG puede ayudar a detectar la patología temprana de la EA, incluido el deterioro visoespacial junto con las preocupaciones de memoria.; Identificar a las personas con deterioro visoespacial subjetivo podría ayudar en el diagnóstico y tratamiento tempranos de variantes atípicas de la EA.
