Objetivo del estudio:

  • Determinar si la puntuación de riesgo poligénico (PRS) para la enfermedad de Alzheimer (EA) modifica los efectos cognitivos de una intervención multidominio en pacientes con MCI.
  • Evaluar si el estado de portador de APOE ε4 influye en la efectividad de la intervención en función del PRS.

Principales métodos:

  • Se calcularon los rangos percentiles del PRS para 284 participantes coreanos utilizando datos genómicos.
  • Se evaluaron los cambios cognitivos utilizando la puntuación del índice de la escala total de la Batería Repetible de Evaluación del Estado Neuropsicológico (RBANS).
  • Se empleó regresión lineal con términos de interacción para analizar el efecto del PRS y el estado de APOE ε4 en los resultados de la intervención.

Principales resultados:

  • La disminución cognitiva, medida por el cambio en la puntuación RBANS, se asoció con un aumento del PRS.
  • El efecto positivo de la intervención multidominio fue significativamente más pronunciado en individuos con alto PRS (tercil 3, p=0.008).
  • Los beneficios de la intervención aumentaron con el aumento del PRS, particularmente en portadores de APOE ε4 (interacción p=0.04).

Conclusiones:

  • La efectividad de la intervención multidominio para el MCI está influenciada por la predisposición genética individual a la EA.
  • Un mayor riesgo genético (alto PRS) y la portación de APOE ε4 amplifican los beneficios de las intervenciones multidominio de estilo de vida.
  • Estos hallazgos respaldan intervenciones personalizadas para la prevención de la EA basadas en perfiles de riesgo genético.
