Sus antecedentes:

  • Una mujer de 66 años se presentó con vértigo subagudo y pérdida de peso significativa.
  • Los antecedentes médicos incluían artritis reumatoide, diabetes, ERGE, hipertensión, ansiedad y depresión.
  • Seis meses después de la evaluación inicial, desarrolló opsoclono, fasciculaciones linguales, dismetría, temblor, hipoestesia y ataxia.

Objetivo del estudio:

  • Investigar la causa de los déficits neurológicos persistentes en una paciente con antecedentes de enfermedades crónicas.
  • Establecer el diagnóstico de un síndrome neurológico paraneoplásico.

Principales métodos:

  • Extensa evaluación diagnóstica que incluyó RM, análisis de LCR, TC y TEP-TC.
  • Análisis inmunohistoquímico de una biopsia de ganglio linfático.
  • Tratamiento con pulsoterapia de metilprednisolona.

Principales resultados:

  • La RM reveló atrofia cerebelosa; el LCR mostró bandas oligoclonales y un índice de IgG elevado.
  • La TC de tórax y la RM mamaria identificaron un ganglio linfático axilar y una lesión mamaria sospechosos.
  • La biopsia confirmó carcinoma de mama metastásico, con marcadores inmunohistoquímicos específicos (CK7+, CK20-, GATA-3+, ER+, PR-, C-erbB-2 indeterminado, Ki-67 50%).

Conclusiones:

  • Se diagnosticó a la paciente de degeneración cerebelosa paraneoplásica secundaria a cáncer de mama.
  • Los síndromes neurológicos paraneoplásicos pueden preceder o seguir al diagnóstico de cáncer, lo que requiere una evaluación integral.
  • El reconocimiento y manejo tempranos de los síntomas neurológicos y la malignidad subyacente son cruciales.
