Objetivo del estudio:

  • Investigar la relación entre los indicadores de exposición a estrógenos y el riesgo de demencia en mujeres posmenopáusicas.

Principales métodos:

  • Se utilizaron modelos de riesgos proporcionales de Cox con datos de 9.793 mujeres posmenopáusicas del Registro Sueco de Gemelos.
  • El análisis incluyó la edad cronológica como escala de tiempo, con la edad de 65 años como punto de partida.
  • Se utilizó la entrada retardada para prevenir el sesgo de supervivencia, con 1.241 casos de demencia durante 14,43 años de seguimiento.

Principales resultados:

  • Tanto la duración del ciclo reproductivo como la edad de la menopausia espontánea mostraron relaciones no lineales significativas con el riesgo de demencia.
  • Mayores desviaciones de la media en estas medidas se asociaron con un mayor riesgo de demencia.
  • Los modelos tuvieron valores de concordancia idénticos (0.57), lo que indica un poder explicativo limitado para la varianza del inicio de la demencia.

Conclusiones:

  • La exposición a estrógenos parece desempeñar un papel en las diferencias individuales del riesgo de demencia.
  • Las medidas actuales (duración del ciclo reproductivo, edad de la menopausia) tienen limitaciones para explicar los vínculos mecanicistas.
  • Se necesita más investigación con medidas integrales de exposición a estrógenos a lo largo de la vida para aclarar la conexión estrógeno-EA.
