Sus antecedentes:

  • La demencia surge de exposiciones a lo largo de la vida, lo que requiere un enfoque integrador para comprender el envejecimiento cerebral patológico.
  • Las interacciones multifactoriales entre los determinantes contribuyen al riesgo de demencia.
  • Se propone un análisis exploratorio para identificar perfiles de riesgo de demencia y marcadores de resonancia magnética cerebral asociados.

Objetivo del estudio:

  • Identificar grupos distintos de individuos basados en factores de riesgo de demencia.
  • Investigar la asociación entre estos grupos y el riesgo de demencia durante cinco años.
  • Examinar la relación entre los grupos y los marcadores de resonancia magnética cerebral, incluidos los volúmenes estructurales y las hiperintensidades de la sustancia blanca (HSB).

Principales métodos:

  • Análisis de clases latentes aplicado a 2257 participantes libres de demencia del estudio Memento.
  • Agrupación basada en factores de riesgo de demencia modificables: nivel socioeconómico (NSE), riesgos vasculares y estilo de vida.
  • Modelos de Cox de riesgos proporcionales para el riesgo de demencia y modelos mixtos lineales para la evolución de los marcadores de resonancia magnética cerebral (estructurales y HSB).

Principales resultados:

  • Se identificaron cuatro grupos de factores de riesgo: bajo nivel socioeconómico/alto riesgo vascular/estilo de vida subóptimo (n=275), nivel socioeconómico mixto/alto riesgo vascular/estilo de vida regular (n=635), alto nivel socioeconómico/riesgo vascular medio/estilo de vida mixto (n=856) y nivel socioeconómico intermedio/bajo riesgo vascular/estilo de vida saludable (n=491).
  • El grupo con bajo nivel socioeconómico mostró un riesgo significativamente mayor de demencia (HR=2,02) y volúmenes cerebrales/hipocampales más bajos en comparación con el grupo con alto nivel socioeconómico.
  • Los perfiles con menor nivel socioeconómico se asociaron con volúmenes cerebrales reducidos, mientras que los perfiles de riesgo vascular influyeron en las lesiones cerebrovasculares.

Conclusiones:

  • El nivel socioeconómico es un factor predominante que influye en el riesgo de demencia, independientemente de la predisposición genética (estado APOE-ε4).
  • Los volúmenes cerebrales estructurales se ven significativamente afectados por un menor nivel socioeconómico.
  • Las lesiones cerebrovasculares están influenciadas principalmente por el perfil de riesgo vascular.
