Objetivo del estudio:

  • Evaluar la rentabilidad de los umbrales de p-tau en plasma (liberal, intermedio, conservador) para la preselección en ensayos clínicos de la enfermedad de Alzheimer (EA).
  • Evaluar el impacto en la reducción de escáneres de PET Aβ y los costos generales del ensayo.

Principales métodos:

  • Se estudiaron 1.673 individuos en todo el espectro de la EA con datos de p-tau217 en plasma y PET Aβ.
  • Se probaron umbrales liberales (95% de sensibilidad), intermedios (índice de Youden) y conservadores (95% de especificidad).
  • Se calculó el tamaño de la muestra de reclutamiento en función del rendimiento del ensayo y la prevalencia de positividad de Aβ.

Principales resultados:

  • El umbral intermedio demostró la mayor rentabilidad, seguido por el umbral conservador.
  • La preselección con el umbral intermedio redujo los escáneres de PET Aβ en un 64% en individuos cognitivamente sanos (CU) y en un 46% en individuos cognitivamente deteriorados (CI).
  • Se observaron reducciones significativas de costos: 59% en CU y 39% en CI, con hallazgos similares en diferentes ensayos de p-tau.

Conclusiones:

  • El p-tau217 en plasma es una herramienta eficaz de preselección para ensayos clínicos de EA.
  • El umbral intermedio ofrece la estrategia más rentable, reduciendo los escáneres de PET Aβ y optimizando el reclutamiento.
  • Este enfoque mejora la eficiencia de los ensayos clínicos para personas de todo el espectro de la EA.
