Sus antecedentes:

  • El método estándar de Centiloide (CTR) para la tomografía por emisión de positrones (PET) de tau ofrece mediciones consistentes entre diferentes trazadores de tau.
  • Esta investigación tiene como objetivo definir los umbrales regionales de CTR para la elevación de tau y explorar su relación con la beta-amiloide (Aβ), la neurodegeneración y la función cognitiva en todo el espectro de la Enfermedad de Alzheimer (EA).

Objetivo del estudio:

  • Establecer umbrales regionales de Centiloide (CTR) para la elevación de tau PET.
  • Evaluar la relación entre CTR, carga de beta-amiloide (Aβ), neurodegeneración y estado cognitivo en todo el espectro de la Enfermedad de Alzheimer (EA).
  • Determinar la prevalencia de tau en el lóbulo temporal medial (MTL) y cortical en poblaciones cognitivamente no deterioradas (CU), con deterioro cognitivo leve (MCI) y con EA clínica.

Principales métodos:

  • Se utilizaron datos de 924 participantes del estudio AIBL-ADNeT, que incluyeron individuos cognitivamente no deteriorados (CU), con MCI y con EA clínica.
  • Se realizaron PET de tau 18F-MK6240, PET de Aβ 18F-NAV4694, RM y evaluaciones cognitivas.
  • Se analizó la correlación entre el tau global y regional (CTR), el Aβ global (Centiloide), la atrofia cerebral y el rendimiento cognitivo.

Principales resultados:

  • Se establecieron umbrales de PET tau de 2 DE: 11 CTR en el lóbulo temporal medial (MTL) y 15 CTR en la corteza temporoparietal (TP) para individuos Aβ-negativos (A-) cognitivamente no deteriorados (CU).
  • Se observó una asociación no lineal entre Aβ y tau, con tau cortical infrecuente por debajo de 60 Centiloides (CL).
  • Se encontró que en MCI y AD A+, el tau cortical se correlacionó inversamente con la edad y el volumen cortical; el tau limitado al MTL afectó principalmente la memoria episódica. En ausencia de tau, Aβ no se correlacionó con el deterioro cognitivo.

Conclusiones:

  • Se establecieron con éxito los umbrales regionales de CTR y se determinó la prevalencia de tau en el MTL y cortical en todo el espectro de la Enfermedad de Alzheimer (EA).
  • Se confirmó que el tau cortical se detecta raramente por debajo de 60 Centiloides (CL) de Aβ.
  • Se demostró que el tau cortical se correlaciona fuertemente con el deterioro cognitivo y la atrofia cerebral, mientras que el tau confinado al MTL se asocia con déficits de memoria leves.
