Sus antecedentes:

  • Las hiperintensidades de la sustancia blanca (HSB) son comunes en personas mayores y en pacientes con enfermedad de Alzheimer (EA).
  • Las causas subyacentes y el impacto cognitivo de las HSB en la EA no se comprenden completamente.

Objetivo del estudio:

  • Investigar la relación entre las HSB (globales y regionales) y las patologías relacionadas con la EA en pacientes con diagnóstico biológico de EA.
  • Explorar la asociación de la carga de HSB con el rendimiento cognitivo, particularmente con las puntuaciones del Mini-Mental State Examination (MMSE).

Principales métodos:

  • Estudio transversal de 170 pacientes con EA con biomarcadores en líquido cefalorraquídeo (LCR), RM y evaluaciones cognitivas.
  • Se utilizaron escalas semiquantitativas para la evaluación de HSB y atrofia; se emplearon modelos de regresión lineal para analizar las asociaciones con la edad, los biomarcadores de EA, las microhemorragias cerebrales (CMB) y los factores de riesgo vascular.
  • Se analizaron las puntuaciones del MMSE frente a la carga de HSB y atrofia, ajustando por factores demográficos.

Principales resultados:

  • La gravedad global de las HSB se correlacionó con una mayor edad, niveles más bajos de LCR Aβ42 y CMB más graves.
  • Las HSB regionales (periventriculares, frontales, parietales) se asociaron con la edad, el grado de CMB y los niveles de LCR Aβ42.
  • Las puntuaciones bajas del MMSE se vincularon con una mayor atrofia temporomedial (ATM) y una mayor carga de HSB periventriculares, independientemente de la edad, el sexo y la educación.

Conclusiones:

  • Las HSB en pacientes con EA, especialmente en las regiones frontal, parietal y periventricular, se asocian con la patología de la EA (LCR Aβ42, CMB), no con factores de riesgo vascular o tau en LCR.
  • Las HSB periventriculares muestran una asociación independiente significativa con el deterioro cognitivo en pacientes con EA.
