  • El estricto confinamiento de Shanghai (abril-mayo de 2022) proporciona un entorno único para estudiar los efectos de la pandemia en la cognición.
  • Los adultos mayores que viven en la comunidad son un foco para comprender los riesgos de deterioro cognitivo.

Objetivo del estudio:

  • Investigar el impacto de la pandemia de COVID-19 en el deterioro cognitivo en adultos mayores que viven en la comunidad.
  • Comparar los cambios cognitivos en residentes de Shanghai antes y después del confinamiento por la pandemia.
  • Identificar los factores que influyen en el deterioro cognitivo acelerado post-pandemia.

Principales métodos:

  • Estudio de cohorte longitudinal (Shanghai Aging Study) de 3792 residentes de ≥50 años (2009-2024).
  • La función cognitiva se evaluó utilizando el Mini-Examen del Estado Mental (MMSE) y pruebas específicas de dominio.
  • Se utilizaron modelos estadísticos (estudio de eventos, diferencias en diferencias, efectos mixtos) para analizar el efecto de la pandemia en el deterioro cognitivo.

Principales resultados:

  • Se observaron declives más pronunciados en las puntuaciones del MMSE en los años posteriores a la pandemia (2022-2024) en comparación con los períodos anteriores a la pandemia.
  • Se encontró un declive significativo del MMSE en los participantes posteriores a la pandemia (Ola 3) en comparación con la línea de base, sin cambios en el período anterior a la pandemia (Ola 2).
  • El declive fue más pronunciado en las cohortes de nacimiento más tempranas, aquellos con biomarcadores de p-tau basales altos, portadores de APOE4, comorbilidades y uso de medicamentos.

Conclusiones:

  • La pandemia de COVID-19 aceleró significativamente el deterioro cognitivo en los adultos mayores de Shanghai.
  • Las personas con condiciones neurodegenerativas preexistentes o problemas de salud mental mostraron una mayor vulnerabilidad.
  • Los hallazgos resaltan el efecto perjudicial de la pandemia en la salud cognitiva de las poblaciones que envejecen.
