Objetivo del estudio:

  • Investigar la asociación entre los factores de riesgo genético para la enfermedad de Alzheimer (EA) y la demencia con cuerpos de Lewy (DCL) y el riesgo de demencia en individuos con trastorno bipolar (TB).
  • Explorar el papel de la portación del alelo APOE ε4 y las puntuaciones de riesgo poligénico (PGS) para la EA y la DCL en el desarrollo de demencia entre pacientes con TB.

Principales métodos:

  • Se utilizaron datos de 2.238 individuos genotipados con TB de la Colección Sueca de Bipolar (SWEBIC).
  • Se identificaron diagnósticos de demencia utilizando códigos del registro nacional de pacientes.
  • Se calcularon puntuaciones poligénicas (PGS) no relacionadas con APOE para EA y DCL y se evaluó la portación del alelo APOE ε4.
  • Se emplearon análisis de regresión de Cox para examinar la relación entre los factores genéticos y el riesgo de demencia, ajustando por covariables.

Principales resultados:

  • La portación del alelo APOE ε4 se asoció significativamente con un mayor riesgo de demencia en individuos con TB (HR: 1.88, p=0.007).
  • Una puntuación de riesgo poligénico más alta para la enfermedad de Alzheimer (AD-PGS) también se relacionó con un mayor riesgo de demencia (HR: 1.34, p=0.01).
  • No se encontró una asociación significativa entre la puntuación de riesgo poligénico para la demencia con cuerpos de Lewy (DLB-PGS) y el riesgo de demencia en esta cohorte.

Conclusiones:

  • La predisposición genética a la enfermedad de Alzheimer contribuye al desarrollo de demencia en individuos con trastorno bipolar.
  • Estos hallazgos arrojan luz sobre los sustratos genéticos compartidos entre la EA y el TB, ofreciendo información sobre la etiología de la demencia en pacientes con TB.
  • Los resultados respaldan el potencial de estratificación del riesgo genético y estrategias preventivas dirigidas para la demencia en la población con TB.
