Objetivo del estudio:

  • Investigar los determinantes sociodemográficos directos e indirectos del diagnóstico temprano y oportuno de la demencia.
  • Explorar las variaciones en el momento del diagnóstico según medidas cognitivas y temporales.
  • Diferenciar los factores asociados con los diagnósticos de demencia 'tempranos' frente a 'oportunos'.

Principales métodos:

  • Se utilizaron datos de referencia del estudio DETERMINED (940 personas con demencia, 698 cuidadores).
  • Se desarrollaron índices para el diagnóstico temprano (gravedad cognitiva, tiempo desde el inicio de los síntomas hasta el diagnóstico) y el diagnóstico oportuno (informes de pacientes/cuidadores).
  • Se empleó un análisis de vías complejas para examinar las asociaciones directas e indirectas de las variables de fondo con el momento del diagnóstico.

Principales resultados:

  • Los determinantes sociodemográficos mostraron efectos variados y a veces contradictorios en las vías de diagnóstico.
  • El diagnóstico temprano de demencia en términos cognitivos fue más común en díadas de blancos; el diagnóstico temprano en términos temporales fue más común en personas no blancas.
  • Se identificaron determinantes distintos para los diagnósticos de demencia tempranos frente a los oportunos.

Conclusiones:

  • El análisis de vías complejas proporcionó nuevas perspectivas sobre los factores sociodemográficos que afectan el momento del diagnóstico de la demencia.
  • La operacionalización del diagnóstico 'temprano' es vital para obtener resultados de investigación consistentes y comprender el momento del diagnóstico.
  • El diagnóstico temprano no significa inherentemente un diagnóstico 'oportuno'; estos términos no son intercambiables en la investigación o la política.
