Objetivo del estudio:

  • Investigar el efecto del control intensivo de la presión arterial sobre los parámetros de rigidez arterial.
  • Examinar la asociación entre la rigidez arterial y el deterioro cognitivo en el ensayo SPRINT.

Principales métodos:

  • El ensayo SPRINT comparó el control intensivo (PAS <120 mmHg) frente al control estándar (PAS <140 mmHg) de la presión arterial.
  • La rigidez arterial (T-PWV, S-PWV, LD-PWV) se midió al inicio y durante 3 años.
  • El deterioro cognitivo (MCI/PD) se evaluó durante 10 años; se realizó un análisis de tiempo hasta el evento.

Principales resultados:

  • La mayor T-PWV y S-PWV basales se asociaron con un mayor riesgo de deterioro cognitivo.
  • El control intensivo de la presión arterial condujo a una disminución del endurecimiento dependiente de la carga (LD-PWV).
  • Las disminuciones en LD-PWV se relacionaron con una reducción del 21% en el riesgo de deterioro cognitivo.

Conclusiones:

  • Los componentes de la rigidez arterial se asocian con el deterioro cognitivo, independientemente del control de la presión arterial.
  • La rigidez arterial es un factor de riesgo pasado por alto para el deterioro cognitivo.
  • El tratamiento de la rigidez arterial puede ser una estrategia para la prevención de la demencia.
