Sus antecedentes:

  • Los inhibidores de la dipeptidil peptidasa 4 (DPP-4i) son fármacos antidiabéticos comunes con posibles efectos neuroprotectores.
  • El impacto de los DPP-4i en la función cognitiva en adultos mayores con diabetes tipo 2 (DM2) sigue sin estar claro.

Objetivo del estudio:

  • Investigar la asociación entre el uso de inhibidores de la DPP-4 y los cambios en la función cognitiva en pacientes ancianos con diabetes tipo 2.
  • Analizar el impacto de los DPP-4i en la memoria, la atención, las funciones ejecutivas y la categorización semántica.

Principales métodos:

  • Estudio longitudinal de adultos con DM2 no demenciados (≥65 años) de la cohorte de Diabetes e Deterioro Cognitivo de Israel (IDCD).
  • Análisis de registros de salud electrónicos para el uso de medicamentos antidiabéticos, clasificando la exposición a DPP-4i.
  • Modelos mixtos lineales mixtos ajustados por factores de confusión para evaluar los cambios cognitivos a lo largo del tiempo.

Principales resultados:

  • No se encontraron diferencias significativas en la cognición global, la atención, la función ejecutiva o la categorización semántica entre los usuarios de DPP-4i y los no usuarios.
  • Se observó una tendencia hacia un declive más lento de la memoria en el grupo de DPP-4i (p=0,06).
  • Se observó una divergencia en la puntuación de memoria en personas mayores de 70 años.

Conclusiones:

  • El uso de DPP-4i no se asoció significativamente con cambios generales en la función cognitiva en esta cohorte de DM2.
  • Una posible tendencia de preservación de la función de la memoria justifica una mayor investigación.
  • Se necesitan tamaños de muestra más grandes y períodos de seguimiento más largos para confirmar estos hallazgos.
