Objetivo del estudio:

  • Evaluar el riesgo de demencia en pacientes con artritis inflamatoria en comparación con una población emparejada.
  • Analizar el impacto de los agentes biológicos en la incidencia de demencia en pacientes con artritis reumatoide.

Principales métodos:

  • Se utilizaron datos del Servicio Nacional de Seguro de Salud para la identificación de pacientes.
  • Se definieron casos de espondilitis anquilosante, artritis reumatoide seropositiva, artritis psoriásica y espondiloartropatía enteropática.
  • Se empleó un emparejamiento por puntuación de propensión 1:5 para la selección del grupo de control y códigos de diagnóstico específicos para la identificación de demencia.

Principales resultados:

  • El diagnóstico de artritis reumatoide seropositiva se asoció con una mayor incidencia de demencia (HR 1.10, IC del 95 % 1.02-1.20).
  • El uso de agentes biológicos se correlacionó con una menor incidencia de demencia (HR 0.46, IC del 95 % 0.33-0.65).
  • Los agentes biológicos no anti-TNF mostraron una reducción significativa en la incidencia de demencia (HR 0.37, IC del 95 % 0.21-0.65).

Conclusiones:

  • Confirma un alto riesgo de demencia en pacientes con artritis reumatoide seropositiva.
  • Demuestra que los agentes biológicos reducen el riesgo de demencia en la artritis inflamatoria.
  • Destaca que los agentes biológicos no anti-TNF ofrecen un efecto protector estadísticamente significativo contra la demencia.
