Sus antecedentes:

  • Los determinantes sociales de la salud (SDoH) están relacionados con la enfermedad de Alzheimer y demencias relacionadas (ADRD).
  • La mayoría de las investigaciones se centran en la ascendencia europea, lo que hace necesarios estudios en poblaciones subrepresentadas.
  • Este estudio investiga los SDoH en poblaciones afroamericanas (AA), mexicoamericanas (MA) y blancas no hispanas (NHW).

Objetivo del estudio:

  • Examinar el impacto de los SDoH en el deterioro cognitivo en diversos grupos raciales/étnicos.
  • Identificar factores de riesgo específicos de la raza/etnia para el declive cognitivo.
  • Informar intervenciones de salud pública específicas para la prevención de ADRD.

Principales métodos:

  • Estudio observacional, basado en la población, de 3229 participantes de la cohorte HABS-HD.
  • Se utilizó aprendizaje automático (eXtreme Gradient Boosting, random forests) para evaluar 37 SDoH y factores de riesgo.
  • Se emplearon explicaciones aditivas de Shapley (SHAP) para la importancia de las variables y análisis estratificados por raza/etnia.

Principales resultados:

  • El ingreso fue un factor de riesgo principal para todos los grupos.
  • Para AA: la educación, la preocupación (PSWQ) y el índice de privación del área fueron significativos.
  • Para MA: la depresión (GDS), la preocupación (PSWQ) y los años en EE. UU. fueron clave.
  • Para NHW: la depresión (GDS) y la salud autopercibida fueron los más importantes.

Conclusiones:

  • Los SDoH tienen una importancia variable para el deterioro cognitivo en las comunidades étnicas.
  • Se identificaron comunidades de alto riesgo basadas en factores específicos de SDoH.
  • Apoya intervenciones personalizadas y específicas etno-raciales que combinan salud pública y manejo de enfermedades para la prevención de ADRD.
